Background There is ongoing debate regarding whether and under which circumstances secondary findings (SF) should be offered in the pediatric context. Although studies have examined patient perspectives on receiving SF, little research has been conducted examining the experiences of clinicians offering SF to parents of newborns receiving genomic sequencing for a recently diagnosed medical condition.Methods To address this, we conducted qualitative interviews exploring the views and experiences of 12 clinicians who offered SF to parents of infants who had diagnostic exome (...) sequencing (ES) to identify the cause of their hearing loss. Interviews explored clinicians’ accounts of parents’ choices and decision-making about receiving SF, their views on whether and when to offer SF, their experiences returning SF, and any ethical challenges they encountered. Interviews were audio-recorded, transcribed and analyzed using inductive content analysis.Results Clinicians reported parents who declined all SF often felt finding out about future conditions unrelated to their child’s hearing loss may be unhelpful, or even harmful, or were overwhelmed by their child’s diagnosis. Clinicians also reported that some parents chose SF because they felt obliged to, even if they did not want to receive them. They explained that while some parents experienced decision-making regarding SF as positive, for others, this process was challenging or distressing. While clinicians generally agreed SF should be offered, mainly to promote parental choice, most felt SF should be offered after disclosing diagnostic results, primarily to avoid overwhelming parents. Clinicians encountered several ethical challenges, including balancing parental autonomy with non-maleficence, wanting to report or not report certain SF, and questioning whether parents can make an autonomous choice regarding SF.Conclusions Our findings, which are novel as they relate to parents of young infants with a recent diagnosis of hearing loss, add new insights into clinicians’ and parents’ decision-making regarding SF in pediatrics. Growing evidence suggests that natural environments – whether in outdoor or indoor settings – foster psychological health and physiological relaxation, indicated by increased wellbeing, reduced stress levels, and increased parasympathetic activity. Greater insight into differential psychological aspects modulating psychophysiological responses to nature-based relaxation videos could help understand modes of action and develop personalized relaxation interventions. We investigated heart rate variability as an indicator of autonomic regulation, specifically parasympathetic activity, in response to a 10-min video intervention in two consecutive studies as (...) well as heart rate. We hypothesized that a nature-based relaxation video elicits HRV increase and HR decrease, with response magnitude being affected by aspects of early life adversity and trait mindfulness. In Study 1, N = 60 participants watched a relaxation video intervention depicting different natural scenery. We analyzed changes in HR and respiratory sinus arrhythmia as a standard HRV measure, both based on 3-min segments from the experimental session, in multiple growth curve models. We found a decrease in HR and increase of RSA during the video intervention. Higher paternal care and lower trait mindfulness observing skills were associated with higher RSA values before but not during video exposure. In Study 2, N = 90 participants were assigned to three video conditions: natural scenery from Study 1, meditation video, or short clip from “The Lord of the Rings.” Again, HR decreased, and RSA increased during video segments, yet without expected group differences across different video types. We found higher parental care and lower parental overprotection to predict higher RSA at different times during the experiment. Interestingly, lower paternal overprotection predicted overall higher RSA. These results suggest a generic relaxation effect of video interventions on autonomic regulation that we discuss in light of different theories mapping restorative effects of natural environments. Further, psychological characteristics like aspects of early life adversity and trait mindfulness could contribute to individual differences in autonomic regulation. This study contributes to a better understanding of autonomic and psychological responses to relaxation videos. (shrink)

To this day, the Italian Jewish literary postwar canon is undisputedly ruled by Primo Levi, Giorgio Bassani and Carlo Levi. This study of three major Italian Jewish women writers – Natalia Ginzburg, Clara Sereni and Lia Levi – highlights the presence in Italian literature of a subversive Jewish écriture feminine. These writers’ formal independence and subversive redeployment of narrative and thematic strategies not only consolidated a strong female voice in Italian literature but also produced a specific Italian brand of (...) Jewish literature. The following analysis shows how, through their literary ‘life miniatures’, Ginzburg, Sereni and Levi recentre the domestic everydayness of woman’s personal and historical experience, while, simultaneously, challenging the traditional representations of women’s positions within family and within the public space, as well as interrogating their Jewish identity vis-a-vis their country’s Fascist past. In particular, this article focuses on the way in which all three authors portray themselves as women trying to strike a balance between their Jewish identity and history alive only in the domestic space of their family lives, and their gender identity which is repressed by the patriarchal system both in the house and in the public arena. These women respond by moving ‘out’ of their homes, by exploring the city space – thus turning the ‘urban monster’ into a positive locus for women’s self-determination and political action – and by bonding with other women. Through a demasculinization of the city space, these memoirs re-elaborated notions of family, Judaism, private and historical memory, and they reinvented a poetics for the ‘small virtues’ of woman’s everyday existence while also pioneering a new space in literature that radically changed the direction of patriarchal Italian culture in the second half of the twentieth century. Si le nom d’Albert Camus continue de s’imposer, aujourd’hui, comme une figure incontournable de la littérature et de la pensée françaises du XXe siècle, il n’en est pas moins demeuré une personnalité cosmopolite, sensible à ce que la culture ne s’accomplit véritablement qu’en l’absence de sectarisme, qu’en présence de l’autre — avec ou envers lui, peu importe. C’est aussi tout le sens de la collection « Exotopies » de l’Association portugaise des études françaises (A.P.E.F.) qu’inaugure ce volume : présenter des (...) travaux sur la langue et la culture françaises, mais d’un point de vue singulier, celui d’un autre pays. En lisant ce bouquet assez restreint, mais en même temps assez diversifié de travaux sur l’oeuvre d’Albert Camus, on relèvera donc qu’il ne s’agissait ni d’un hommage, ni d’une commémoration, si contraires au vif esprit de la littérature, mais de s’aviser de l’exceptionnelle variété et actualité de son oeuvre. En ce sens, il n’est de meilleure figure pour débuter cette collection à visée cosmopolite que celle d’Albert Camus, chez qui le geste d’écriture est totalement inscrit dans une vocation humaniste. Celle-ci ne s’entend pas dans un sens moraliste, mais dans un sens existentiel — sinon existentialiste —, qui veut que chacun se définisse par le souci qu’il a de l’autre, et que l’humanité s’apparaisse comme le perpetuum mobile de l’histoire, en même temps que son point fixe, son sens dernier. La psicología contemporánea parece caracterizarse desde sus propios orígenes por la multiplicidad de sus contenidos, además de por su casi infinita fragmentación en corrientes encontradas. Esto crea importantes dificultades, no sólo a quienes quieran tener una primera aproximación, sino también a los especialistas, que muchas veces no llegan a una opinión suficientemente clara sobre la naturaleza epistemológica de la psicología, ni sobre su unidad disciplinar. Esta obra, sin descuidar el problema global, se centra en un aspecto particular: el que (...) presenta la praxis de la psicología, en particular la psicoterapia y sus fundamentos teóricos. Se pone especial interés en una visión crítica del psicoanálisis, punto de referencia inevitable (positivo o negativo) para la mayoría de las corrientes de psicoterapia. Pero este intento de esclarecimiento epistemológico y práctico se lleva a cabo desde un punto de vista original: a la luz del pensamiento de Santo Tomás de Aquino (1225-1274), conocido como uno de los más grandes teólogos y filósofos de la Iglesia Católica. Developing and applying a framework for understanding the complexities of economic and legal considerations in two recent Supreme Court rulings was the focus of this research. Of especial concern was the protection of intellectual property in the pharmaceutical industry. Two cases from 2013 were selected: FTC v. Activis and Association for Molecular Pathology v. Myriad Genetics, Inc.. Part of the rationale for the selection was the importance of the Supreme Court rulings and the importance of the pharmaceutical sector. A qualitative (...) content analysis of the Court’s reported decision in each case was analyzed. Since ethical considerations may or may not be consistent with efficiency considerations of plaintiffs, defendants, or the courts, both efficiency and ethical arguments were included. Equally important to the understanding of the economic and ethical issues in the two above- mentioned cases was the development of a rationale for including and excluding a variety of ethical theories, ultimately influenced by Markkula Center For Applied Ethics Of Santa Clara University and Schumann :93–111, 2001). The refinement and use of a levels analysis approach portrays societal, organizational, and individual impacts, and allows for deeper understanding of litigated cases, irrespective of the country in which the litigation takes place :385–393, 2004; Foss et al., J Manag Stud 47:455–482, 2010). A rationale for this framework is the societal and organizational impacts on the myriad of stakeholders in the pharmaceutical sector. We suggest that this framework can be applied to other industries and other complex conflicts among stakeholders as well. (shrink)

El propósito de este artículo es el de justificar una interpretación genuinamente hegeliana de la dimensión religiosa en el pensamiento de Hegel, particularmente a partir de la Fenomenología del espíritu. Tanto la izquierda como la derecha hegeliana desarrollaron un carácter unívoco, o bien reduciendo el concepto hegeliano de lo divino a la razón natural o histórica, o bien pasando por alto que la concepción hegeliana de una divinidad inmanente resultaba incompatible con el cristianismo ortodoxo. Analizaremos en detalle los capítulos de (...) la Fenomenología del espíritu que se centran en la Ilustración, en los cuales el autor se desmarca de forma más clara de lo que podría ser una religiosidad puramente deísta. Finalmente, la propuesta de Hegel consistirá en un modelo orgánico de estado, dentro del cual la religión sería aceptada como parte de la misma substancia ética que fundamenta las leyes de cada comunidad. The globalization in business and tourism becomes crucial more and more for the economical sustainability of local communities. In the presence of an epidemic outbreak, there must be such a decision on the policy by the host community as whether to accept visitors or not, the number of acceptable visitors, or the condition for acceptable visitors. Making use of an SIRI type of mathematical model, we consider the influence of visitors on the spread of a reinfectious disease in a community, (...) especially assuming that a certain proportion of accepted visitors are immune. The reinfectivity of disease here means that the immunity gained by either vaccination or recovery is imperfect. With the mathematical results obtained by our analysis on the model for such an epidemic dynamics of resident and visitor populations, we find that the acceptance of visitors could have a significant influence on the disease’s endemicity in the community, either suppressive or supportive. With your eyes again fixed on the speaker, and the movement of her lips a clear sense of the source of the sound will (...) return. This ‘ventriloquist’ effect reflects the ways in which visual cognition can dominate auditory perception. And this phenomenological observation is one what you can verify or disconfirm in your own case just by the slightest reflection on what it is like for you to listen to someone with or without visual contact with them. Background A morally sound framework for benefit-sharing is crucial to minimize research exploitation for research conducted in developing countries. However, in practice, it remains uncertain which stakeholders should be involved in the decision-making process regarding benefit-sharing and what the implications might be. Therefore the study aimed to empirically propose a framework for benefit-sharing negotiations in research by taking HIV vaccine trials as a case. Methods The study was conducted in Tanzania using a case study design and qualitative approaches. Data were (...) collected using in-depth interviews (IDI) and focus group discussions (FGD). A total of 37 study participants were selected purposively comprising institutional review board (IRB) members, researchers, community advisory board (CAB) members, a policymaker, and HIV/AIDS advocates. Deductive and inductive thematic analysis approaches were deployed to analyze collected data with the aid of MAXQDA version 20.4.0 software. Results The findings indicate a triangular relationship between the research community, researched community and intermediaries. However, the relationship ought to take into consideration the timing of negotiations, the level of understanding between parties and the phase of the clinical trial. The proposed framework operationalize partnership interactions in community-based participatory research. Conclusion In the context of this study, the suggested framework incorporates the research community, the community being researched, and intermediary parties. The framework would guarantee well-informed and inclusive decision-making regarding benefit-sharing in HIV vaccine trials and other health-related research conducted in resource-limited settings. The ancient and honorable role of philosophy as a servant to the learning, development and use of scientific knowledge, though sadly underdeveloped since Grassmann, has been re-emerging from within the particular science of mathematics due to the latter's internal need; making this relationship more explicit (as well as further investigating the reasons for the decline) will, it is hoped, help to germinate the seeds of a brighter future for philosophy as well as help to guide the much wider learning (...) of mathematics and hence of all the sciences. 1. The unity of interacting opposites "space vs. quantity", with the accompanying "general vs. particular" and the resulting division of variable quantity into the interacting opposites "extensive vs. intensive", is susceptible, with the aid of categories, functors, and natural transformations, of a formulation which is on the one hand precise enough to admit proved theorems and considerable technical development and yet is on the other hand general enough to admit incorporation of almost any specialized hypothesis. Readers armed with the mathematical definitions of basic category theory should be able to translate the discussion in this section into symbols and diagrams for calculations. 2. The role of space as an arena for quantitative "becoming" underlies the qualitative transformation of a spatial category into a homotopy category, on which extensive and intensive quantities reappear as homology and cohomology. 3. The understanding of an object in a spatial category can be approached through definite Moore-Postnikov levels; each of these levels constitutes a mathematically precise "unity and identity of opposites", and their ensemble bears features strongly reminiscent of Hegel's Science of Logic. This resemblance suggests many mathematical and philosophical problems which now seem susceptible of exact solution. (shrink)

This paper explains how to use a new software tool for argument diagramming available free on the Internet, showing especially how it can be used in the classroom to enhance critical thinking in philosophy. The user loads a text file containing an argument into a box on the computer interface, and then creates an argument diagram by dragging lines from one node to another. A key feature is the support for argumentation schemes, common patterns of defeasible reasoning historically know as (...) topics . Several examples are presented, as well as the results of an experiment in using the system with students in a university classroom. (shrink)

Robert Kimball, in “What’s Wrong with Argumentum Ad Baculum?” (Argumentation, 2006) argues that dialogue-based models of rational argumentation do not satisfactorily account for what is objectionable about more malicious uses of threats encountered in some ad baculum arguments. We review the dialogue-based approach to argumentum ad baculum, and show how it can offer more than Kimball thinks for analyzing such threat arguments and ad baculum fallacies.

For 20th century mathematicians, the role of Cantor's sets has been that of the ideally featureless canvases on which all needed algebraic and geometrical structures can be painted. (Certain passages in Cantor's writings refer to this role.) Clearly, the resulting contradication, 'the points of such sets are distinc yet indistinguishable', should not lead to inconsistency. Indeed, the productive nature of this dialectic is made explicit by a method fruitful in other parts of mathematics (see 'Adjointness in Foundations', Dialectia 1969). In Morals By Agreement, David Gauthier concludes that under certain conditions it is rational for an agent to be disposed to choose in accordance with a fair cooperative scheme rather than to choose the course of action that maximizes his utility. This is only one of a number of important claims advanced in that book. In particular, he also propounds a distinctive view concerning what counts as a fair cooperative arrangement. The thesis concerning the rationality of adopting a cooperative disposition (...) is, however, logically independent of his substantive view of a fair cooperative scheme and is itself central to the project as a whole. Gauthier's concern is to establish that certain moral principles are those that fully rational, self-interested persons would agree to take as regulative of their dealings with one another – that a contractarian approach, in this sense, can provide an adequate basis for a theory of morality. This paper explores ethical debates associated with the UK COVID-19 contact tracing app that occurred in the public news media and broader public policy, and in doing so, takes ethics debate as an object for sociological study. The research question was: how did UK national newspaper news articles and grey literature frame the ethical issues about the app, and how did stakeholders associated with the development and/or governance of the app reflect on this? We examined the predominance of different ethical (...) issues in news articles and grey literature, and triangulated this using stakeholder interview data. Findings illustrate how news articles exceptionalised ethical debate around the app compared to the way they portrayed ethical issues relating to ‘manual’ contact tracing. They also narrowed the debate around specific privacy concerns. This was reflected in the grey literature, and interviewees perceived this to have emerged from a ‘privacy lobby’. We discuss the findings, and argue that this limited public ethics narrative masked broader ethical issues. (shrink)

Whether a predicate is a referential expression depends upon what reference is conceived to be. Even if it is granted that reference is a relation between words and worldly items, the referents of expressions being the items to which they are so related, this still leaves considerable scope for disagreement about whether predicates refer. One of Frege's great contributions to the philosophy of language was to introduce an especially liberal conception of reference relative to which it is unproblematic to suppose (...) that predicates are referring expressions. According to this liberal conception, each significant expression in a language has its own distinctive semantic role or power, a power to effect the truth-value of the sentences in which it occurs. I argue that the concept of 'longitudinal form', which Poplawski and Gillett have introduced into ethical discussions about embryos and gametes, involves too many metaphysical subtleties to be a useful aid to making moral decisions. I conclude by suggesting a criterion for relevance in medical ethics.
